What is Otoplasty?
Otoplasty is a cosmetic ear surgery that improves the shape, position, or proportion of one or both ears. Most often, the procedure is performed to correct prominent ears by setting them closer to the head. Depending on the degree of correction needed, surgery typically takes 45 to 60 minutes per ear.
Otoplasty is often chosen by individuals who:
- Feel their ears stick out more than they would like
- Believe their ears are too large in proportion to their head (macrotia)
- Are dissatisfied with the results of a previous otoplasty performed elsewhere
- Have a birth defect or injury that has altered the appearance of their ears
What is "Prominent Ear Deformity"?
Prominent ear deformity, also called protruding ears, typically involves a combination of three structural irregularities. The severity of these features varies from person to person, and an otoplasty procedure may address one, two, or all three during surgery. These characteristics include:
- An absent antihelical fold
- An obtuse conchoscaphal angle
- A deep conchal bowl
Together, these irregularities create a greater than normal distance between the helical rim of the ear and the scalp, which makes the ears appear more prominent.
How Otoplasty Is Performed
Otoplasty can be done using local anesthesia, sedation, or general anesthesia. For pediatric patients, general anesthesia is usually recommended to ensure comfort and safety. The procedure begins with an incision on the back of the ear, allowing the surgeon to access and reshape the cartilage.
Depending on the desired changes, cartilage may be reshaped, repositioned, or, in some cases, partially removed. Permanent sutures are typically placed to hold the cartilage in its new position. As healing progresses, scar tissue naturally forms, helping maintain the ear's new contour. Otoplasty can be performed on one or both ears as needed.
Incision Placement
In most cases, the only incision required is located on the back of the ear, near the crease where the ear meets the scalp. This location allows the incision to heal discreetly and remain well hidden. In some situations, a small incision—usually 5 to 8 millimeters—may be made on the front of the ear to allow access to the cartilage surface for additional refinement.
Post-Operative Care
Following otoplasty, discomfort is generally mild to moderate. Patients are prescribed pain medication and antibiotics to aid in comfort and healing. A protective head wrap is applied immediately after surgery and worn for the first 24 hours.
On the following day, this wrap is replaced with a lighter, mild compressive headband worn for one to three days. After that period, the headband is only required at night for approximately two weeks to protect the ears during sleep and prevent accidental pulling while turning in bed.

Generally, a child or young adult has more flexible or malleable ear cartilage. This allows for easier manipulation of the ear and, many times, a more natural appearing result. As we age, the cartilage that gives shape to the ear tends to get harder. When ear cartilage becomes stiff, it may require scoring or cutting to shift the ear into a more aesthetically pleasing position.
Studies show that the best time to perform an Otoplasty is around five years old. The ear is almost its mature adult size, and the cartilage is very malleable. The child will be entering school at this age. Having prominent ears tends to elicit teasing from other children. Intervening at this stage in the child's life can preserve their psychological wellness. Females can grow their hair long to hide their protruding ears however males typically wear their hair short.
